How it is calculated

Bicarbonate deficit (mEq) = (base deficit × body weight in kg) ÷ 4 [1].

Conservative formula: 0.4 × body weight (kg) × (12 − measured HCO₃⁻) = replacement dose in mEq [1].

Give only a quarter to a half of the total calculated dose and repeat blood gases in 5 to 10 minutes [1].

Worked example

20 kg, base deficit 12: 12 × 20 ÷ 4 = 60 mEq (give 15–30 mEq first).

Frequently asked questions

How much sodium bicarbonate should I give?

Bicarbonate deficit (mEq) = base deficit × body weight (kg) ÷ 4; administer only a quarter to a half of the total calculated dose and repeat blood gases in 5 to 10 minutes [1].
